Unsuccessful Sign Off1 Attempt and Unsuccessful Sign Off2 Attempt occur during attempts to perform result signoffs when an incorrect password is entered.
Unsuccessful Attempt to Confirm Identity can occur depending on policy settings where the option to Confirm Identity has been enabled. This would apply based on System Audit Trail settings in System Policies. This also applies to fully audit trailed projects where Confirm Identity has been enabled for Method, Result, Sample, and Deletion events.
Unsuccessful Logon Attempt can occur at initial login, unlocking locked Empower application screens.
