ANSWER

The "Date", "Time", "Date/Time of Print", and "Timezone of Print" metadata is taken from the system clock on the machine from which the report is printed.

The "Date/Time of Database Insertion" and "Timezone of Database Insertion" metadata is generated by the NuGenesis Web server when the report is inserted into the database.
