ANSWER
When a SmartBuilder form contains a named input field having Reason for Change enabled, a change is made to the contents of that input field and the Reason for Change is entered. "Motivation for Change" is populated in the document's "Audit Trail Report" but is left empty in the "Document with Audit Trail Report". Motivation for change should be populated with the same value in both of these reports.
